Contact our First Contact team to self refer
The first point of contact for people who have a diagnosis of a long-term or life-threatening condition.
The service helps people to understand and access the wide variety of services available through the Hospice and in the community.
Calls are welcomed from patients and their families and also from doctors, hospitals and community staff.
Contact us
The team can introduce the new patient and their family to a range of services, always making sure they are chosen to meet each person’s individual needs.
This could include:
- a drop-in session
- advice and support
- a home visit
- nursing care
