Referrals are processed by the Referrals Office which is open 9am to 5pm Monday to Friday.

Telephone advice and support
Our telephone number 01284 766133 provides advice to health professionals (e.g. GPs, community nurses, care homes) and families on symptom management and end-of-life care.
Our specialist teams will continue to support as usual during the day, but will increasingly offer telephone advice. Our aim is to ensure people are equipped and prepared, and that if things change unexpectedly, additional expert advice is available.
What we do
The specialist expertise of the Hospice staff includes:
• Specialist palliative inpatient hospice care (short stay)
• Knowledge and skills in resolving uncontrolled or complicated symptoms that have not responded to first or second-line treatments
• Managing short-term complex therapy needs
• Facilitating psychological, emotional, spiritual social or family issues in conjunction with other agencies
• Bereavement support for adults, children and young people
• Supporting the patients and families with difficult decision-making in future end-of-life care provision
• Support from experienced volunteers

The process after referral
We will contact the GP surgery to inform the GP of the referral and to request medical information in the form of copies of letters and scan reports.
We then share electronic records with other healthcare professionals. The Hospice Referrals team will contact the healthcare professionals who are already involved where appropriate and also contact the patient/family/carer.
This will help us to ensure the patient’s first assessment is allocated to the most appropriate team (which may include external agencies) and is carried out in a timely fashion according to need and priority. This first assessment may be undertaken at home or virtually depending on the situation. The Referrals team will also be able to signpost or refer on to other services where necessary.
