Patient Safety: Our Commitment to Learning and Improvement
At St Nicholas Hospice Care, we are committed to continuously improving the safety and quality of our care. As part of our alignment with the national NHS Patient Safety Strategy, we have adopted the Patient Safety Incident Response Framework (PSIRF) to guide how we respond to and learn from patient safety incidents.
Our approach is rooted in compassion, transparency and system-wide learning — not blame. We aim to ensure that every incident is treated as an opportunity for improvement and that patients, families, and staff are supported throughout.
Our Key Documents
To ensure transparency and inform stakeholders, we are publishing the following documents:
• Patient Safety Incident Response Framework Policy STNH649
This policy outlines our overall approach to patient safety, including roles, responsibilities, and how we plan and evaluate responses to incidents.
• SNHC Patient Safety Incident Response Framework plan
This plan provides specific details on how we will implement PSIRF in practice, including our priorities, governance structures, and response processes.
Both documents are reviewed annually or in line with national guidance to ensure we continue to meet the highest standards of safety and care.
