Out of Hours Community Palliative Care Team Lead

St Nicholas Hospice Care is a local charity providing help, advice and support to people in West Suffolk and Thetford who are living with long-term and life-threatening illnesses. Our services are not just for patients, but for their family and friends too. They include everything from specialist medical care to bereavement support and practical help. St Nicholas Hospice is dedicated to its vision that everyone in our communities has support, dignity and choice when facing dying, death and grief.

An exciting new opportunity has arisen to join our Out of Hours (OOH) Community Palliative Care Responsive Team as the OOH Community Palliative Care Team Lead. The Out of Hours Community Palliative Care Team is now part of our established multi-disciplinary team, consisting of clinical nurse and paramedic specialists, senior hospice nurses and health care assistants providing responsive specialist support to patients and families with a life-threatening illness in our local community overnight.

The successful applicant will lead the OOH community hospice nursing team, influencing the holistic care of patients and carers with end of life and specialist palliative care needs. This is a chance to make a real difference to people’s lives.

You will work within the OOH Community Team providing specialist palliative care advice and support to patients in West Suffolk and Thetford. You will provide both clinical and day to day management to the OOHs Clinical Nurse/paramedic Specialists, Registered Nurses and Healthcare Assistants within the Community Team and be expected to support the ongoing development of the palliative care community responsive team. You will also work with the wider multi-disciplinary team providing face to face and telephone support, for the responsive needs of patients and participate in answering calls from our telephone advice line.

Hours: 37 or 30 hours per week considered.

The successful candidate will be expected to demonstrate a high level of expertise both clinically and managerially, have excellent communication skills and the ability to work flexibly with consideration to the needs of the service.
