Community Hospice Team
A group of professionals and trained volunteers provide end of life care and support in wherever you call ’home’, including care homes, prisons and sheltered housing. This support is available to patients and carers throughout West Suffolk and Thetford.
We work closely with your GP and District Nurses so that they remain your main contact for in the community. The Team co-provides end of life care with the NHS, voluntary organisations and social care agencies.
The Team recognises that each person’s situation is different and we provide a range of support to reflect this, always delivered by the most appropriate professional.
The team is made up of:
Clinical Nurse Specialists
Help you with important decisions, planning your care and managing symptoms.
Provide hands-on care in the last weeks of life. Working with the Clinical Nurse Specialists, GPs and District Nurses, they can help to prevent unnecessary admissions to hospital and be supportive to family members.
Supporting People Team
Social Workers, Psychological Specialist and Chaplains can help family carers or friends who are finding it hard to cope.
Our Physiotherapist, Occupational Therapist and Complementary Therapists are trained to maximise patients’ rehabilitation potential regardless of disease to maintain a sense of control and adaptation.
Offer specialist medical advice and work closely with GPs.
Support the Community Hospice Team by providing support for carers and giving companionship.
